RE:                 HEALTH & PHYSICAL FORMS FOR THE
                       2008-2009 SCHOOL YEAR

Enclosed please find the following health forms for your child to be completed for the School Health Office for the 2008-09 school year.

1.         Asthma Action Plan – If your child has a history of asthma, contact Mrs. Maxwell for an 
                                                Asthma Action Plan.
2.         Athletic Pre-Participation Physical Form

            (This is a MANDATORY EXAM for all seventh graders.)

            Also, please complete for those students in grades 6 and 8 who wish to participate in the      
            extracurricular sports program. 

Return as soon as possible.  This form may be downloaded from the link on the LMS website labeled Athletics Information or by copying the following link into your browser http://www.state.nj.us/education/districts/forms.htm .  For those parents who do not have E-mail, please contact Mrs. Maxwell to have a form sent to you.

3.           EMERGENCY PLAN – All grades please complete this form.

4.           MEDICATION FORM – If your child needs to take medication during school hours, please contact Mrs.Maxwell so that a form can be sent to you. Your physician must also complete his/her portion of the form.
