How to Register

Follow the Online Registration Directions.

Required Sports Physicals (2025–2026)

If your student plans to participate in any athletic program for the 2025–2026 school year, the forms below must be completed before participation in any practice or game:

  • Preparticipation Physical Evaluation — Medical Eligibility Form (required; return to the school nurse)
  • Preparticipation Physical Evaluation — History Form (completed and maintained by your doctor; do not return to school)

Leonia High School Athletics — Fall Registration

Registration is now open for all fall athletic programs at Leonia High School. Please complete online registration and required medical forms before the first practice date.

  • Girls Soccer

Upcoming Sports Physical Deadlines

  • Winter Athletic Season

    • Leonia High School: November 17, 2025 (first group of programs)
    • Leonia High School: November 24, 2025 (remaining programs)
    • Leonia Middle School: December 15, 2025

 

  • Spring Athletic Season

    • Leonia High School: March 9, 2026
    • Leonia Middle School: March 30, 2026

 

All sports physical forms must be completed and submitted to the school nurse by the deadlines listed above. Students will not be permitted to participate in practices or games without current and approved documentation.
